At the School of Youth Ministries in English (SYME) in Korea we have developed a tool that we use in evangelistic outreach. This tool is a 10-question survey that helps us understand where people are in their thinking about God and spiritual things. It helps us "knock on the door" to their heart and life and to naturally engage in conversation about some of the most important issues in their life.

In Korea (and in Japan) our students will go out into major population areas (near subway stops). We look for people who are sitting, standing, waiting and appear to have a couple of minutes to take our survey. We go up to them, introduce ourselves and explain that we are students at SYME. In our school we are required to do 10 surveys. We ask if they could help us with this assignment.

We explain that we are taking a Short Spirituality Survey that asks 10 questions. We show them the survey. Each question has 4-5 possible answers including one they can add. The answer by circling one of 4-5 pictures.

The questions are as follows:
1. How much do you think about God or spiritual things? (never-seldom-sometimes-often-always)
2. How would you describe your spiritual life/journey?
(growing-stopped-shrinking-deciding-other)
3. Has church/religion helped/hurt you much on this journey?
(much hurt-hurt-none-helped-much help)
4. Which religious symbol best describes you?
5. How much do you think God is involved in the lives of people?
(never-seldom-sometimes-often-always)
6. What kind of person do you think Jesus was?
(legend-crazy-good-God-other)
7. What do you think happens when our life on earth ends?
(nothing-reincarnation-heaven/hell-don't know-other)
8. What do you wish God would do for you?
(protect-guide-save-bless-other)
9. Are you interested in these topics?
(yes-don't know-no)
10. Would you like to discuss these issues more?
(yes now-yes by e-mail-yes in a small group-don't know-no)

If at the end they say "yes-now" then we turn the survey over and there is a 5-minute presentation on what the Bible says about how to have a relationship with God.

If at the end they say "yes by e-mail" then we ask for their name and e-mail address.

If at the end they say "yes in a small group" then we get name, e-mail and phone number and invite them to a local study or event.

If at the end they say "I don't know" we show them the back side of the survey and explain that we could show them what the Bible says about how to have a relationship with God. Then we ask if that is OK. If they say yes, we continue.

If at the end they say "no," we don't continue.

We tell everyone that we will be totaling the results of our survey and if they would like to receive these results of what everyone answered we could send that to them in an e-mail. If they want this we make sure to get their e-mail address.

But for everyone we give them a comic tract we have created with an interesting testimony and gospel message. We also usually give them a treat (gum or candy) with this and thank them for taking the time to do our survey.

This version has been translated into Japanese and Korean. We will try to post those versions here at a later date. Also, if you can translate this into another language I could send you the MS Word file so you could translate and change it easily. Then please let us know and allow us to post these other versions on this site as well. May this be used for His kingdom and glory.

If you are not a Christian and have come across this site I would encourage you to download this survey and take it yourself. Especially read the second side of the survey form. I believe you will find it very thought provoking.
